Male infertility
This problem is very different from impotence or erectile dysfunction! In this case, a man with infertility problems may be able to maintain an erection but may not be able to father a child due to problems with sperm count or other factors. How and who determines the sperm count and other factors? The answer is an Andrologist! The andrologist is the specialist who deals only with male problems such as: infertility and problems in the sexual field, namely: erectile dysfunction, impotence, premature ejaculation and curved penis or Peyronie’s disease.
How is a diagnosis made?
It is necessary to perform an examination of the sperm (ejaculate), and laboratory diagnostics of male infertility followed by the visit of a specialist, the Andrologist. But be careful to consider yourself a problem, a couple is defined infertile after 12 months of regular unprotected sexual intercourse in the absence of conception. Following that, proper verifications on both the woman and the man can be performed. For men, analyzing sperm is essential for getting answers. In men, the analysis of sperm is highly variable, this is due to the fact that the cycle of spermatogenesis (the formation and formation of a mature sperm) occurs within 72-74 days.
An exam to be repeated!
Being a consistent period of time, many events can occur that have a negative effect on spermatogenesis. For example: inflammatory diseases with high fever, intoxication and the intake of numerous drugs. Various emotional and physical overloads, smoking, alcohol intake and many other reasons can have a negative effect. This negative effect may be temporary, therefore, if the first analysis showed deviations from normal values, it is necessary to repeat the analysis after 2 weeks. It is possible to judge the state of spermatogenesis only by several analyzes of sperm, and if deviations from the norm are determined in the analysis, but motile spermatozoa are found, it is necessary to talk not about complete male infertility, but about a reduced probability of pregnancy.

Male infertility treatments!
If the sperm analysis turns out to be abnormal, your andrologist will determine the possible causes and appropriate treatment. Infertility treatment in men,has the goal to improve sperm quality and it is necessary for the subsequent start of an independent pregnancy or to increase the likelihood of pregnancy. Spermatogenesis is a very complex, sequential and multistage process, which involves a huge number of biochemical reactions, which in turn are controlled by more than 200 genes. And if something goes wrong at any stage or in any reaction, the quality of the sperm gets worse.. The more accurately the cause of spermatogenesis failure is identified, the greater the success of treatment for male infertility.
Spermatogenesis
It is a very sensitive process, which depends on the work of all the organs and systems of the body. The causes and methods of treatment of deviations are quite diverse, but sometimes it is enough to prescribe health programs, quit smoking, stop alcohol abuse and other bad habits, as noted by the improvement in sperm quality. To improve the quality of sperm, drugs and preparations are used that act on the various stages of sperm formation and on the function of the accessory genital organs.
Important factors for spermatogenesis, lifestyle:
For both men and women, ovulation and spermiogenesis (the production of healthy sperm) can be severely affected by certain environmental factors and lifestyle choices. These include:
- Obesity: Having a BMI of 30 or more reduces fertility for both men and women. Obesity also has an impact on IVF success rates and increases the risk of complications during pregnancy; for both mother and baby
- Infections: Chlamydia and other sexually transmitted infections can reduce fertility. ● Smoking: Smoking tobacco reduces sperm quality and can adversely affect egg quality. ● Alcohol Consumption: Excessive alcohol consumption, as well as recreational drug use, can adversely affect fertility.
- Environmental Hazard Exposure: Environmental hazards and industrial toxins, such as pesticides and solvents, can damage fertility, as can some medications such as anabolic steroids and corticosteroids.
What can the Holistic Andrology Clinic and Dr Castiglione do?
Dr Castiglione deals with male infertility and the Holistic Andrology Clinic boasts a staff of certified andrologist-urologists recognized throughout the world for their clinical and academic skills. The main purpose of our work is the selection of subjects in which surgical or medical therapies can give a stable seminal improvement and, therefore, a good chance of spontaneous conception in a reasonably short time. An essential component of this work is to provide the couples under examination with accurate and truthful information about the realistic expectations of success of the proposed therapeutic programs. The conduct of our specialists aims at improving seminal parameters (number, motility and morphology of spermatozoa): their careful evaluation using unique formulas allows us to approximate the probabilities of spontaneous conception in the couple under examination.

A couple’s diagnosis
In the assessment of couple infertility, the characteristics of the female partner must also be considered, which can strongly influence the success rates of any male treatment (for example, the age of the woman, a previous already documented fertility, normal or pathological tubal patency , endometriosis, etc.). When the conditions that allow a reasonable probability of spontaneous fertilization do not exist, timely recourse to low technology assisted fertilization (homologous intrauterine insemination = IUI) or high technology (IVF or ICSI) may be necessary. When it is impossible to change too poor sperm quality with both medical and surgical therapies, assisted fertilization (ICSI, for the most part) remains the only possible treatment.
N.B.
In azoospermic subjects (i.e. with the absence of spermatozoa in the seminal fluid – Azoospermia) or unable to ejaculate (for psychological or neurological reasons), it is possible to recover spermatozoa from the seminal tracts to be used for fertilization with surgical or microsurgical or instrumental techniques minimally invasive assisted (ICSI).
Advanced diagnostics
At the Clinic we can perform sperm function tests. The TUNNEL, SCSA and HALO test allows to identify cases of alteration of the spermatic DNA (fragmentation of the spermatic DNA): it is useful when the number and motility of sperm detected by the program is standard.
The MAR-Test allows the detection of the presence of anti-sperm antibodies on the surface of the sperm.
- Bacteriological examination.
- Scrotal ecocolordoppler.
- Prostate-vesicular ultrasound.
- Genetic analysis: (BLOOD EXAMINATION) the karyotype allows to identify any chromosomal irregularities.
- Testicular biopsy: to estimate the functionality of the TESTICLE.
