Dr. Fabio Castiglione is both an Andrologist and Urologist. He graduated in Medicine and Surgery and subsequently completed his training in Urology at the Vita Salute San Raffaele University in Milan. Most of the published studies are based on animal models, but there was also case series. In one of this study, the patients, all diabetics, aged between 57 and 87, received treatment with intracavernous injections based on stem cells taken from the umbilical cord. Six out of seven men experienced morning erections six months after the injection. With the concomitant use of Viagra®, all of these patients were able to achieve vaginal penetration. No side effects have been reported. Stem cells have evidently been shown to promote angiogenesis, revascularization and improve blood flow. Dr. Castiglione had the idea to use stem cells to treat a disorder of the penis known as Peyronie’s disease when he was 24 years old. His intuition: to utilize stem cells to minimise fibrosis in the penis (one of the reasons of both penile curvature and erectile dysfunction) has become the subject of worldwide research, making Castiglione one of the most prominent references in this field. Research on rats gives an abundance of solid data that stem cells can be applied to humans. In addition, some human trials have been undertaken (as stated in the first paragraph), but not enough to consider it a standard treatment. Consequently, it can only be categorised as experimental for the time being. The concept of employing stem cells for this type of treatment is based on their capacity to function as a “pharmacy.” If they are administered to an appropriately inflamed system, they can decrease collagen production. Diabetes, ageing, and cardiovascular disease all contribute to an increase in penile fibrotic tissue. Due to collagen (fibrotic tissue), the penis is unable to extend adequately, resulting in erectile dysfunction and a condition known as bent penis (Peyronie’s disease).
They can enter the system and affect this biological circuit, lowering fibrosis while improving vascularity. The “beauty” of stem cells is that they travel to the site of the issue. This means that, in this case, they do not have to be put into the penis: the stem cells travel where there is inflammation and, if there are enough of them, they can reverse the system by generating younger tissue. This is what happens with so-called “lipofilling,” which is utilised by many plastic surgeons to treat scars, for example. The root of the disease is not eliminated in this manner, therefore it is not a definitive intervention; however, in a process of general improvement of the patient’s conditions, the help of these stem cells brings the penis into a state that can have a long-term benefit.
Currently, there are more than 15 clinical trials, it has been proven that they are safe but to be able to demonstrate their effectiveness, a number and careful selection of patients is necessary. There are studies that have collected 20-30-40 patients who have been given stem cells and a positive effect has been observed. But the power calculation of the study, i.e. the number of patients involved in it, was not such as to make it statistically significant.
